Who we are

One organization, wrapped around the whole person.

Future Hope Outreach Foundation is a community health organization rooted in the neighborhoods it serves. We started at the pharmacy counter — and quickly learned that a prescription only works when a person also has a safe place to sleep, support for their mental health, and a path toward stability.

So we built partnerships instead of silos. We work alongside street medical teams, supportive housing sites, mental health programs, substance-use and MAT providers, HIV-prevention programs, and re-entry organizations to close the gaps that fall between traditional services — the gaps where people are most often lost.

Today that work spans four connected efforts: our Skid Row wraparound care model, an emerging East LA wellness clinic, Prescribe Your Future — a workforce pipeline creating pharmacy careers — and multilingual medication access for refugee and newcomer families.

    DMC-ODS provider partner

    A Drug Medi-Cal Organized Delivery System (DMC-ODS) contracted SUD treatment provider in LA County — the partner that unlocks Medi-Cal reimbursement and county incentives for the people we serve.

  • Clinical

    MAT / MOUD prescribers

    Licensed clinicians who can assess for opioid use disorder and prescribe buprenorphine and naltrexone, so patients can start evidence-based treatment without delay.

  • Partner

    Opioid Treatment Program (OTP) for methadone

    A SAMHSA-certified OTP willing to accept referrals, so patients who need methadone have a clear, coordinated path to it.

  • Partner

    340B / FQHC covered-entity partner

    A federally qualified health center or 340B covered entity to partner with our pharmacy network — dramatically lowering medication costs for uninsured patients.

  • Funding

    Community Health Worker funding

    Support to grow our Community Health Worker team — the trusted, on-the-ground connectors who keep patients engaged in care.

  • Funding

    Grant partners & funders

    Funders and grant-writing partners to help cover the cost of SUD medications for uninsured patients across every clinic we serve.
